I know the difference in appearance, but was there a difference in application?
Was the 50 for an M38 and the 50R for M38A1?
Or was it a running change based on the need for more pulling capacity?

To the best of my Knowledge the only winch fitted by Willys was the 50. All the 50R's were the standard replacement in the field. It would be nice to get a confirmation which winch the Canadian M38's and A1's had installed in the field there.
