Correction, one side the key was on the shaft when the hub was installed, the other side, after the hub was installed.

You need to be sure of your answer. Placing the key in the slot first is a no-no. It leads to binding of the hub on the shaft and the hub will not slide fully on the axle.
I would suggest, since you memory of the installation is not too sharp, go back and remove the hub and key. Make sure the axle and the inside of the hub are clean and have a light coat of anti-seize on them. Slide the hub all the way on and make sure it is fully seated. Align the hub and axle key way. Now tap the key into the slot. Now you can install the washer and torque the nut.

Thanks. My Dad did the re-assembly, I helped him but, I spent more time arguing with my dad to go by the book so I wasn't paying attention. Your help is greatly appreciated.
